Fill out and then scan and e-mail back to nmebasketballgroup@gmail.com Please note that the "Player Profile" must be filled out and provided to NME staff on or before participating in any NME event. So even if you are trying out for the first time, and some of the questions may be repetitive, both forms must be filled out and turned in. Other pieces of information such as a birth certificate and insurance card will be required.
